US issued Plastic Mastercards. They are re loadable with accounts attached.
Card limits are:
Monthly max load $7,500.
Daily ATM withdraw limit $1,000.

You can use card with Paypal if you have US Paypal or if your Paypal supports withdraw to US account.
You can not get the expuse number to verify Paypal. You need to verify your account with another card.

Price - $150. Shipping varies by method desired.

Yes, your name is printed on the card, delivery varies with your location but 10 to 14 days should cover all areas. yes you can order more than one.
