For sure this is a scam. Whatever you do, do not pay them anything and most definitely not up-front.

As I see the form does not contain too much personal info, no bank data or even passwords. So I think the risk is low. You'll probably receive a lot of SPAM to your e-mail address.

Answer to your post
Yes, I am familiar with this email and is a scam. I researched it last year after replying to my email, which was identical. Through my investigation I found that it is a postal scam! You can not be hurt by the information you gave; however you will be given instructions to pay for the shipping of your funds that you will never recieve. These con-artist make fortunes through shipping cost.

Report phishing emails to Gov
I also recieved semail very recently saying I won a huge amt. in a UK lotto, to 2 totally different emails.
I have had several of the Nigerian emails claiming to I have un-claimed money from a deceased relative. Never reply.
Always report.
Here is the Official US Government page. OnGuard Online - Phishing
It will tell you all kinds of info & exactly where to forward to, etc. etc. which btw- Forward to the email to:
[email protected]
p.s. I would go ahead & do this no matter where you live, since these e-mails most likely come from out-side US anyway. or put into search for your country========================================
