What is the Reason for keywords not Ranking in Google?

Have you optimised your site with proper titles, alt tags, header tags, placed that keywords in content for needed amount for each page, have made some inner linking and linkbuilding as well as solid external one, contacting lot of sites for link opportunity and manually build yours ? What was the results ?
